Health Appraisal Questionnaire

If the link doesn't work you can simply, type (https://haq.metagenics.com) into the Address section of your Internet Browser (Internet Explorer is preferred over Netscape Navigator). 

To Log in:

• You must first have been assigned a login and password.  You will receive an email from info@AnswersToHealth.com listing your login and password.  If not, email (info@AnswersToHealth.com) us so you can be register on the web site, and then go to On-line Health Appraisal Questionnaire (left side of front page).  Your login and password are the same for both entries.  See below for instructions. 

• You will be assigned a login and password. You will receive an email from info@AnswersToHealth.com listing your login and password.  If not, email (info@AnswersToHealth.com) us so you can be register on the web site.  

• You must enter in the login and password exactly as shown. The login/passwords are case sensitive. For example, if your login is john doe, it is not the same as John Doe or johndoe.

• If you have problems logging on, the most likely culprit is that the login or password was not entered correctly. If you are entering the information in exactly as shown, please contact our office at info@AnswersToHealth.com or 303-774-9737.
